What to bring with you:
A part of your immigration physical exam is verifying that your vaccinations are current. To avoid multiple visits and unnecessary vaccination costs, please bring:
- Your completed I-693 form
- Your current and childhood vaccination records, including proof of a COVID-19 vaccination
- Translations for non-English documents
- If you need any additional testing or vaccinations, we can provide blood work, X-rays and vaccinations during your visit
Also, bring:
- Official photo ID, such as a passport, a driver’s license or a state ID
- Your USCIS number (A- number), if applicable
- Any chest X-rays from within the last year
- A translator, if you don’t speak English
After your immigration medical exam, trü Primary & Urgent Care will prepare a report using USCIS guidelines. You will receive the report in a sealed envelope. Do not open the envelope. You must deliver the report to the consulate or USCIS unopened.
The USCIS now requires certain applicants filing Form I-485 (Application to Register Permanent Residence or Adjust Status) to include Form I-693 (Report of Immigration Medical Examination and Vaccination Record) with their application. If Form I-693 isn’t submitted, the I-485 application could be rejected.
