After your immigration medical exam, truHealthNow will prepare a report using USCIS guidelines. You will receive the report in a sealed envelope. Do not open the envelope. You must deliver the report to the consulate or USCIS unopened.
The USCIS now requires certain applicants filing Form I-485 (Application to Register Permanent Residence or Adjust Status) to include Form I-693 (Report of Immigration Medical Examination and Vaccination Record) with their application. If Form I-693 isn't submitted, the I-485 application could be rejected.
